Multidisciplinary Care

Our team works closely with your provider to develop an individualized care plan to meet your recovery goals. You will receive one on one care throughout the course of your treatment.

Scheduling

Our friendly office staff will assist you with scheduling your Outpatient Rehabilitation appointments. We offer flexible scheduling options. For your convenience, we offer services between 7am and 6pm depending on the location and the day of the week. Please mention your scheduling preferences when you call for an appointment.

Your first visit will be scheduled for one hour to allow your therapists to fully evaluate your condition. Follow-up appointments are generally scheduled for 30 minutes. You will be able to discuss the frequency and duration of your treatment with your therapist during your first visit. Please have your doctor's referral/prescription available when you call to schedule your appointments. We can be reached at 617-499-5011.

How to Prepare For Your First Visit

Before arriving to your first appointment, you will need a prescription for outpatient therapy from your referring provider. You may also need to contact your Primary Care Physician to obtain an insurance referral. Before your first appointment, verify with your insurance carrier what is included in your coverage and benefits for therapy, including necessary insurance referrals, deductibles and co-pays. If you need financial assistance, contact the billing department to understand your options.

When You Arrive For Your First Visit

Bring your insurance card, copayment, and prescription from your physician to your first visit with our office. Wear loose clothing to allow access to the area of the body being evaluated. To evaluate your upper body injury you should wear a tank top or T-shirt and for lower body evaluation wear shorts or sweatpants. At check in we will ask you to confirm your contact information and complete a questionnaire and patient waiver form, or, you may print and complete them prior to your arrival.
