Acute Care Hospital Phase 2 Reopen Attestation Form

During the COVID-19 public health emergency to date, in order to ensure Massachusetts’ health care system had the capacity to address the pandemic, some health care services and procedures needed to be limited or put on hold temporarily, while many services were conducted remotely using telehealth. Emergency services and other essential health and human services continued during this time.

On June 6, the Commonwealth issued new guidelines allowing health care providers at the start of Phase 2 to incrementally resume in-person elective, non-urgent procedures and services, including routine office visits, dental visits, and vision care, subject to ongoing compliance with public health and safety standards. All other in-person medical, behavioral health, dental and vision services may resume, except for elective cosmetic procedures and in-person day programs, which will be a part of Phase 3. Find more information on reopening plans, phases, guidance, and forms on this page. Commonwealth's Reopening Website.

Mount Auburn Hospital has submitted the Acute Care Hospital Phase 2 Reopen Attestation Form to the Massachusetts Department of Public Health (DPH) on June 8, 2020. By having submitted this form, Mount Auburn Hospital attests to meeting the Phase 2 criteria and standards in the DPH Hospital Reopening Guidance.
