Get medical care at home with Mount Auburn Hospital’s HouseCalls. Through this program, physicians visit older adults who are homebound or returning home after staying in a hospital or rehabilitation facility.
You or a loved one will benefit from care that focuses on managing chronic conditions and multiple medications, and avoid hospitalizations.
Who Qualifies for HouseCalls?
If you’re over 65 years of age and homebound, you may be eligible for HouseCalls. Medicare considers you homebound if:
- You need the help of another person or assistance from medical equipment to leave your home OR your doctor believes your health or illness could get worse if you leave your home
- It’s difficult for you to leave your home and you typically can’t do so
Comprehensive Geriatric Assessments
Your first HouseCall visit includes a comprehensive assessment of your or your loved one’s physical and mental health from a geriatrician—a doctor who specializes in caring for older adults. Assessments focus on issues that affect your or your loved ones’ quality of life and daily functioning. They may include screenings for:
- Behavioral health
- Disease risk factors
- Fall risks
- Memory problems
- Nutritional deficiencies
- Vision and hearing problems
Assessments also consider current medical conditions and medications as well as potential preventive care.
Senior Health Care at Home
To help you or your loved one manage chronic conditions and facilitate independent living, HouseCalls physicians will develop individualized long-term care plans. They work closely with caregivers, including any home care visiting nurses, to coordinate and supervise ongoing care at home.
